Code of Practice for Data Portability

When healthcare providers switch from one Healthcare IT system to another, it is essential that they continue to have access to the Records of their patients for continuity of care and patient safety. This necessitates accurate and complete porting of Records from one Healthcare IT system to another.

The Ministry of Health (MOH), in consultation with relevant stakeholders, has developed the Code of Practice for Data Portability (“Code”).
This Code is intended to:

  • Set out the practices that Healthcare IT Vendors shall comply with in the provision of support or services to ensure Data Portability;

  • Promote a Client-centric approach to the provision of Data Migration services to ensure Data Portability; and

  • Advocate best practices for Data Portability across the Healthcare IT industry.

This Code applies to all Healthcare IT Vendors that provide the following Healthcare IT systems:

  • Clinic Management Systems (CMS) for Private Primary Care under the CMS Tiering Framework for Primary Care (link); and

  • IT systems for Active Ageing Centres under the Age Well SG (ASG)-compatible AAC IT System Qualification Framework (link).

For any further clarification or assistance, Healthcare IT Vendors can reach out to Synapxe at the following contacts:

General Practitioners and Community Care Organisations should direct their queries to their respective AIC account managers.
