Infectious Disease Notification

Medical practitioners and laboratories are required to notify MOH of specified infectious diseases under Section 6 of the Infectious Diseases Act and events of public health significance.

Educational institutions should report clusters of illness (such as two or more people ill with similar symptoms closely grouped in terms of time and place).

How to submit infectious disease notifications?

Infectious Disease Notifications can be submitted via the following means:

• Electronic Notification System (CD-LENS)
  https://www.cdlens.moh.gov.sg/cdlens

OR

• Filling up the following hardcopy notification forms and faxing to: 6221 5528 / 6221 5538 / 6221 5567
