Infectious Disease Notification
Medical practitioners, laboratories and educational institutions should submit infectious disease notifications to Ministry of Health.
Under Section 6 of the Infectious Diseases Act (IDA), medical practitioners and laboratories are required to notify MOH of all confirmed and suspect cases of infectious diseases in the Schedule and within the prescribed time.
Besides individual cases of notifiable infectious diseases, medical practitioners must continue to notify MOH of events of public health significance (e.g., clusters of infectious diseases, suspected bioterrorism agents, suspect cases of emerging infectious diseases) immediately.
Educational institutions should notify MOH of any infectious diseases outbreak of the following:
Gastroenteritis / Food poisoning
Hand, Foot and Mouth Disease
Acute Respiratory Illness
Other significant disease
As a guide, an infectious disease outbreak can be defined as 2 or more linked cases of the same illness or when the number of cases of the same illness unaccountably exceeds the expected number. Please refer to the Infection Prevention Guidelines for Schools and Child Care Centres for more information on the control measures and examples of infectious diseases commonly reported by schools.
How to submit infectious disease notifications?
Note: With the launch of the new CDLENS portal on 12 April 2025, we have decommissioned the previous CDLENS portal. We have also phased out fax notifications. User guides for the new CDLENS portal can be found here.
All infectious disease notifications should be submitted via the following modes:
Electronic Notification System (CDLENS)
https://de-rom.moh.gov.sg/cdlens
OR
Existing institution systems interfacing with CDLENS (e.g., EPIC NGEMR, SingHealth Sunrise Clinical Manager, SmartCMS systems)
OR
In case of system(s) downtime where submission via the above modes are not possible, medical practitioners, laboratories and educational institutions can fill up the following pdf notification forms and email to cdlens_notification@cda.gov.sg.
