Primary care is often the first point of contact for patients, addressing their acute and chronic care needs. Primary care services include but are not limited to:
Management of chronic conditions as part of Healthier SG, such as diabetes, high cholesterol, and high blood pressure, as well as follow-ups on Health Plan
Treatment of mild acute conditions such as common cold/flu and gastric discomfort
Treatment of infectious diseases such as Chickenpox and Hand, Foot, and Mouth Disease
Childhood immunisations, growth monitoring and developmental progress checks
Adult vaccinations
Coordination of patient care with other care providers, especially for patients who need help to navigate the healthcare system or referral to a specialist
GP Clinics
There are more than 2,000 private GP clinics in Singapore of which more than 1,300 are Community Health Assist Scheme (CHAS) clinics. CHAS card holders can receive subsidies for medical care at CHAS clinics.
To ensure holistic, team-based care for patients with chronic conditions such as diabetes, hypertension and hyperlipidaemia, over 1,000 CHAS GP clinics, including 8 Family Medicine Clinics (FMCs), are organised into 10 Primary Care Networks (PCNs).
PCNs are networks of General Practitioners (GPs) supported by a team of nurses and care coordinators. FMCs are multi-doctor practices that are supported by a team of nurses and allied health professionals, providing consultation and support services under one roof. GPs may also refer their patients to Community Health Centres (CHCs) for ancillary services such as diabetic eye and foot screening, and nurse counselling.

There are 26 polyclinics in Singapore. These provide subsidised primary care, which includes:
