MediSave may be used for premium payment. Premium recovery measures may be taken for those who do not pay despite multiple reminders.

Using MediSave to pay MediShield Life premiums for yourself and your loved ones
MediSave may be used to pay MediShield Life premiums in full, to minimise cash payment. There are measures in place to help Singapore Citizens and Permanent Residents use their MediSave to pay for their premium payments. These include:
Regular reminder letters to top up your MediSave account for premium payment
You can top up your MediSave account to pay the premiums via E-Payment (E-Services @ CPF)Use of family members’ MediSave accounts for premium payment
If you have insufficient MediSave savings, your immediate family members, such as your parents or spouse, may apply to CPFB to help with your premium payment. The new payer will be informed in writing before any deduction from their MediSave account is made, and may review the arrangement at any time.
If you wish to help pay your loved one's MediShield Life premiums, please submit your application by using E-Services @ CPF(Singpass login required).
Late Payment / Non-Payment of MediShield Life Premiums
The Government provides a range of premium subsidies to help you pay your MediShield Life premiums. Additional Premium Support is available if you cannot afford your premiums even after Government subsidies, MediSave use and have limited family support. No one will lose coverage due to an inability to pay.
However, for those who have the means but choose not to pay their MediShield Life premiums despite the multiple reminders, a strong set of premium recovery measures will be taken.
The premium recovery measures include:
Appointment of defaulter’s agents (such as the defaulter’s employers, banks and tenants) to recover the outstanding premiums
Offsetting outstanding premiums against one-off Government surplus-sharing transfers (e.g. Growth Dividends)
Recovery from CPF monies that are withdrawn in cash
Imposition of travel restrictions in certain circumstances
Legal action to recover the arrears
Penalties will be imposed on those who do not pay. Those who persistently delay premium payment may also be required to pay back any foregone interest to the MediShield Life Fund.
Appointed under the MediShield Life Scheme Act, the Inland Revenue Authority of Singapore (IRAS) and the CPF Board are recovery bodies responsible for imposing premium recovery measures.
